To obtain a Right-of-Entry permit, you must submit a completed application which includes the following documents:
– A current certificate of insurance naming the Mayor and City Council of Baltimore as an additional insured
– A Certificate of Good Standing from the State of Maryland Department of Assessments & Taxation (SDAT)
– A copy of the Articles of Incorporation or Organization of the organization or entity requesting access;

Additional information required to be entered into the application are:
– All dates for which access to the property is requested
– Begin & End times for each date for which the organization is requesting to be on the property (e.g., 8:00am to 12:00pm)
– All locations on the property where activities are to take place, including (if necessary) detailed, marked-up map(s) showing precise location(s)
– Number of participants (best estimate)
– Description of the activities taking place
